Doctor gave us a print out for eMedical Information sheet which has our pictures and UMI number. Also he gave us consent form and test order sheet.
Official site is talking about uploading either the information printout sheet or the Upfront Medical Report form [IMM 1017B
Doctor said he already uploaded our test results. Should I ask for IMM 1017B or just upload the eMedical Information sheet?
